Story of a patient with acute abdominal pain | MARC #804
Hi there! Assalamu Alaikum. How are you? I am hoping that you are well. I am fine as well, Alhamdulillah. Here are my activities on the 27th of August 2022. It was Saturday. It is my Actifit report number 804! In this post, I have included the following things:

Chamber updates
On Saturday, as usual, it was a busy time at my office. Being the single GP doctor, I worked for 10 hours and a half, instead of the regular daily 9 hours. I consulted 61 patients in the evening. In the visa medical section, 45 people came for the check-up. Alhamdulillah, there were no emergency cases.
Story of a patient with acute abdominal pain
One Pakistani guy came with an Egyptian (foreman). The patient was having quite severe pain in his lower abdomen. The pain was associated with a urinary burning sensation and vomiting for a few episodes. During the physical examination, I found the maximum pain in his right lower abdomen. From the history and physical assessment, my differential diagnoses were acute appendicitis and right renal stones.
I told them the possibilities and asked them to do at least 3 tests to reach the diagnosis; a complete blood count (CBC), a urine microscopic test and an Ultrasound of the whole abdomen (scanning). As the scanning wasn't available in my clinic, I gave them two options;
Do CBC and Urine test in my clinic. If reports come in favour of acute appendicitis, go to a surgeon immediately.
Go to a surgeon first and do everything there.
They opted for the second option and left.
They returned within one hour. (Most likely due to high cost) They could not see a surgeon. They wanted to do the available tests in my clinic. So, I ordered the test.
The test results didn't help me reach a unifying diagnosis. The blood test result was in favour of acute appendicitis and the urine report was in favour of renal stones. I explained it to them and asked them to do a scanning anyhow. They agreed. However, they wanted something to reduce the pain. Because they only could do the scanning after 5-6 hours from that point in time. So, I gave him injectable painkillers to reduce his pain. I gave a requisition paper to do the scanning. I also explained to them that if the scan report (ask the sonologist) shows the features of acute appendicitis, see a surgeon there and if the report shows the features of renal stones, come back to me.
They haven't come back yet!
