When we talk about self-defense, it seems it is all about some specific techniques and you do those techniques. That's all. To learn self-defense, you need to learn how to fight. Self-defense techniques will be useful to protect yourself. But there is no way you can ignore physical fitness.

If you are not fit physically, you cannot apply those techniques properly and use them to defend yourself. You have to use your force and strength in a specific way and in a certain direction to make self-defense moves.

Let's talk about a specific self-defense technique. What if someone grabs you from the front side? Let's watch this video. When someone grabs you like this, your both hands are open. Most of the time, if the attacker is a guy and he grabs a girl like this, he can pick the girl up. Then it would be more difficult to get rid of this grip.

As soon as someone grabs you, put your weight down, then you can make your move. You can do what showed in that video. To apply the technique, you have to take a step and put your leg behind his leg. The key point is, if you are not able to make the attacker off-balance, you can make him fall on the ground. You can use one hand to push, the attacker will fall on the ground.

How about if you cannot apply this technique?

Your hands are open, so you can directly strike him. You can do foot stomp. If you use your heel to hit the attacker's foot, this is going to be very painful. You can hit to the groin.

No one is invulnerable at the base of nose, no matter their size or strength. This move however should only ever be used in true self-defence scenarios. A hard blow here can kill someone.

If I can avoid it, I do not want to go down to the ground with an assailant.

Self-defence is about maiming your opponent so you can get away. You do not want to engage them in ground grappling if you can avoid it, even if you are a high-level practitioner of the grappling arts. What if they have a friend? Virtually, every time you take someone down, they will take you down with them. This takes little skill. Gravity does most of the work. Even when I was a yellow belt, I was able to take down a black belt as he took me down.

Your move would be more useful for a law-enforcer or to apprehend or restrain someone. Provided you know what to do next. Or in a sports combat scenario.

This move does not strike me as a go-to self-defence move and would be virtually useless with someone who out-weighed you significantly, regardless of your gender or level of training or strength. There is little movement in a bear hug and throwing one off balance usually means finding the sweet spot in their movement where they are already off balance and maximizing your speed/movement to produce the force and surprise needed.

This is a move for sparring, competing, or restraining.

Eye gouging can be done in this situation if it turns into a very serious situation. In case of a kidnapping or raping attempt, you will have to stop the attacker anyhow.

Yes, when you are going to make the attacker fall on the ground, he can grab you and take you to the ground as well. So need to be careful about that. I completely agree with you. It will better not to get on the ground and be in a grappling situation.

Sometimes I share videos (video link) to understand the situation I am talking about. With due respect, I don't always think what showed in the video would be effective for self-defense. I share alternatives that someone can do in that situation or even if someone uses that technique what they should pay attention to.
