When we fail to reach a diagnosis!😥🤔

Sometimes it happens. A patient comes with a symptom or a group of symptoms but despite doing relevant (available) investigations and examinations, we fail to reach a unifying diagnosis. We either send him/her to the higher center or give some initial symptomatic managements. Today I got two such patients; one was a new case to me and the other one was a returning case.

A middle-aged patient came with abdominal pain. But on physical examination, I found his heart rate abnormally low. When the heart rate of te normal person should be above 60 beats per minute, his heart rate was 38 only! Surprisingly, he wasn’t having any symptoms of this low heart rate. His only complaint was upper abdominal pain (which was not severe though). Having found such a low heart rate, I couldn’t just ignore it. I explained to him (and his attendant) the necessity of evaluating the cause of such abnormal heart rate. They agreed. I referred him to the cardiologist in our clinic. The cardiologist did ECG, Echo and some blood tests. He found no specific reasons behind such a low heart rate!

Another patient came with a urinary burning sensation. He came with a file with many prescriptions and investigation papers. Two years before he came with a similar problem. A urine examination didn’t reveal any features of infections. He then went to his native place and consulted multiple urologists. No clear-cut diagnosis was made by any of the urologists! Today I repeated another urine test. The same result. No features of infections. I had to give him some symptomatic managements only!

We all have our limitations. We can’t solve all the problems! We can’t treat all the patients!!

Today I consulted around 30 patients in my chamber. In the visa medical section, a similar number of people came for the check-up.
